When completing your registration, you will be asked to affirm the following statement.

I understand that if anyone in our household exhibits any of the following symptoms when we are scheduled to visit that we will reschedule:

• temperature higher than 100.4F (or 38C)
• chills
• cough
• shortness of breath or difficulty breathing
• new loss of taste or smell
• runny or stuffy nose (that is unrelated to seasonal allergies)
• sore throat
• nausea
• vomiting or diarrhea
• muscle aches, body aches or headaches (other than those resulting from strenuous activity, exercise or chronic medical or physical conditions)
