My signature below is authorization that I agree to meet the following eligibility requirements to transfer:
· Complete the program inquiry form, available on both OCC’s and SU’s websites.
· Complete an Associate of Arts or Associate of Science program at OCC in the semester immediately preceding the desired entry term at SU.
· Complete my degree with a minimum GPA of 3.0 (a minimum 3.4 GPA is required for admission to the Whitman School of Management). SU and OCC will determine by mutual agreement the programs I am eligible to enter based on my GPA at the time of transfer.
· If I graduate with a GPA of 3.25 or above and apply to SU according to the guidelines outlined in this agreement, I will be awarded a minimum of $10,000 in merit-based scholarship aid to attend SU upon acceptance into a full-time degree program. This aid will be renewable subject to normal SU requirements for merit aid. If I graduate from OCC with a GPA of 3.25 or higher and matriculate into CPS part-time programs, I will receive merit aid in the amount of $5,000. I will also be evaluated for need-based financial aid if I submit a CSS-PROFILE and FAFSA documents.
· Remain in good standing with OCC throughout my enrollment to remain eligible for this Direct Transfer Admission Program.
· This agreement pertains to undergraduate programs at Syracuse University in the following schools:
o College of Arts and Sciences
o College of Engineering and Computer Science
o School of Information Studies
o Whitman School of Management
o College of Professional Studies

· Meet all Syracuse University admission requirements, including completion of an application and written personal statement along with submission of official transcripts from all institutions attended.
· Meet residency and graduation requirements of OCC according to the student catalog of my year of entry.
· Comply with and will be bound by the standards, policies, guidelines and regulations of SU in effect at the time of my application for admission and during my enrollment at SU.
FERPA STATEMENT:
The Federal Family Educational Rights and Privacy Act (FERPA) of 1974 protects the privacy of student educational records, including transcripts, by placing certain restrictions on the disclosure of that information. As a result, your written authorization is required in order for Onondaga Community College to release your educational records to facilitate the transfer agreement.
My signature below is authorization and agreement that:
· I understand the FERPA statement and agree to my student records being shared between Onondaga Community College and Syracuse University for the purpose of credit evaluation to determine the awarding of an associate degree and the intent to transfer.
· If applicable, an appropriate associate degree will be awarded based on my records, requirements of the degree, and credits toward degree.
· I further affirm that the transcripts submitted to Syracuse University accurately represent my academic history and that any falsification or misrepresentation of information may result in the revocation of my admission or disciplinary action, in accordance with policies and regulations of Syracuse University.
· I understand that the release of my transcripts does not guarantee the transfer of credits and that the college will evaluate and make decisions regarding the acceptance and transferability of my coursework in accordance with their policies and procedures.
· I acknowledge that providing this consent is voluntary, and I have the right to revoke it at any time. However, I understand that the revocation of this consent may result in delays or limitations in processing of my application and evaluation of transfer credits.
· I understand that this agreement will not apply if I choose to matriculate at SU prior to the completion of my associate degree. In the event that I transfer prior to graduating from OCC, SU will consider me for admission and financial aid on terms similar to all transfer applicants. In addition, SU will collaborate with OCC on the reverse transfer of credits so that OCC to award me the appropriate associate degrees.
